 Instructions for Recipients of Financial Aid
2005-2006

All of the following materials must be taken to Associate Dean’s Office at Keating 221. Please access the Human Resources Website at:
http://www.fordham.edu/campus_resources/administrative_offic/human_resources
if you have any questions. Your stipend will be paid in 19 bi-weekly installments (9/2/05-5/12/06).

Please fill out the following forms as accurately as possible and submit all of the paperwork by August 15, 2005. Since ID information is required (see #5 below) for the payroll to be processed, please submit the proper ID information in person by no later than August 22, 2005, at 12:00pm. If paperwork is completed properly your first paycheck will be ready September 2, 2005. Please realize that your paycheck will be delayed for the following reasons:
  • Tax forms are not filled out properly
  • Proper ID not submitted as requested in #5 below


1. Graduate Assistant Payment Request Form: Please complete the top half of the form with your local address. If you do not have a local address yet, you can use the department or program you are entering.

2. Direct Deposit Application: We urge you to have your stipend directly deposited into a bank account. If you do not yet have a bank account that you want to use for this purpose, you can submit this form at a later date.

3. Form W-4 (2005): This form is used to figure withholding for federal income tax. If you have questions about your allowances or withholding, consult a tax advisor.

4. Form IT-2104:
This form is used to figure withholding for New York State income tax. If you have questions about your allowances or withholding, consult a tax advisor.

5. Form I-9, Employment Eligibility Verification:
Please read the instructions on this form carefully because incorrect information will delay the start of your pay. You should complete the top half of the form; the bottom half must be verified by Fordham University.

  • Proper ID: You should consult the back of the sheet for the list of documents acceptable for such verification. If at all possible, you should use one document from List A (e.g., U.S. passport) for the verification. If you do not have a document from List A, then you must use one document from List B and one document from list C; two documents from List B are not acceptable.
  • Original acceptable documents should be brought in person to the Associate Dean’s Office in Keating 221

For foreign students only: Please note that we cannot process payroll until you have a U.S. social security number. Foreign students should contact the Office of International Students and supply phone number and e-mail address (Faculty Memorial Hall 428 at the Rose Hill campus or 33 W. 60th Room 306 at the Lincoln Center campus) for more information.

Tuition: To process your tuition scholarship, you must follow departmental guidelines to register by July 7, 2005. We will post your tuition award to your student account after you register.

Fulltime status: As a financial aid recipient, you must be a full-time student (12 credits). If you register for fewer than 12 credits (e.g. because of comps or departmental requirements), you must file a Matriculated Student Status Certification Form every semester. Failure to file this form each semester may result in problems with loan deferments and eligibility. We would appreciate your timely submission of this form so that your tuition scholarship can be ready on time. Please note that in some departments the average course load may comprise less than 12 credits. Please consult your department advisor to determine the required course load for your department.

Instructions for Recipients of Tuition Scholarships
2005-2006


Tuition: To process your tuition scholarship, you must first register by July 7, 2005 . We will post your tuition award to your student account after you register.

If you receive a bill that includes an error in tuition, you must notify the Associate Dean’s Office by the second week of the semester or you may lose your tuition scholarship. Email:

Full time status: As a financial aid recipient, you must be a full-time student (12 credits). If you register for fewer than 12 credits (e.g. because of comps or departmental requirements), you must file a Matriculated Student Status Certification Form every semester. Instructions for Recipients of Fellowships
2005-2006


Stipend: Your fellowship is designed to foster your professional development through an individualized program of research, study and teaching preparation. Because the fellowship stipend is not payment for services, the stipend will be paid through our accounts payable office (rather than through payroll). Your fellowship may not be taxable.

Checks: You will receive nine monthly payments around the 15th of each month,starting in September. Fellowship checks are picked up by your department each month.

Instructions for Recipients of Supplemental Awards and 2nd year fellowship recipients
2005-2006

As the recipient of a prestigious financial aid award, your stipend has two components: payment related to duties and a supplement fellowship. The amount that is related to your duties as a graduate assistant is equivalent to a regular stipend, and will be processed through payroll.
