Immunizations and Health Papers


New York State Law requires that all students born on or after Jan. 1, 1957 submit documentation of immunization against Measles, Rubella and Mumps. Vaccinations must have been given after your first birthday and must include 2 shots against Measles and one for Mumps and Rubella. You must bring official documentation of these vaccinations and dates with you, in English, or translated, to present at registration. You may not be allowed to register without these documents. If you cannot provide specific vaccination information, you may receive vaccinations, for a minimal charge, from the Univity's Student Health Center. The Health Center also offers free Tuberculosis testingers.
