Fordham IT VPN - Lotus ID Request Form

To receive a Lotus Notes client CD, you must complete and submit this form. The information you provide is necessary to complete configuration of the Lotus Notes client.

Fordham IT will send via e-mail, the Lotus Notes ID and Password files necessary to complete your Lotus client configuration. You will be required to unzip the e-mailed files. Instructions will be provided with the Lotus Notes client CD.
