DOCUMENTATION GUIDELINES
In order to receive accommodations for a disability, you must first submit current documentation signed by an appropriate professional to the Office of Disability Services (ODS). Documentation criteria vary based on the nature of the disability. Please select the guidelines that match the nature of your disability (a list can be found on the left-hand side).
Rose Hill and Westchester students should submit their documentation to the Rose Hill office, located in the basement of O’Hare Hall, by mail or via fax: (718) 817-0888.
Lincoln Center students should submit their documentation to the Lincoln Center campus, located in room LL 207, by mail or via fax: (718) 817-0888.
