Please download or copy and or paste this, fill in the relevant information on each line in the same file and send it both by E-mail and regular mail. This asks for basic information about the participants, which is obviously needed even for participants who will not be paying the registration fee. We are waiving the registration fee for participant presenters who are unable to secure funding from their respective employers or institutions. 1] FILE NAME: Name the file as FLLLLLL.txt, where F=your-first-initial and LLLLLL is your last name You can use an abridged version of the name, without hyphens, spaces, etc. For example if your name is John Smith, F=J and LLLLLL=Smith. Then save the file as “JSmith.txt”. The rent for additional days may be paid in advance or upon arrival in NY. The decision on papers accepted for presentation is conveyed to the speakers separately by March 30, 2005. This information is requested on this registration form once again, for our convenience. We need item 4 information about co-authors even if they are not attending the conference. 7]. REGISTRATION FEES (payable to “Fordham University EHR05”). If your payment is postmarked on or Before March 15, 2005 you pay US$100 for regular participant, US$30 for a full-time student and US$30 for accompanying person(s). All registration fees increase by US$5 per month on April 15 ($105), May 15($110), June 15($115) and July 15($120). Seating is limited and places fill on a first-come first served basis. Full-time students will have to include a photocopy of her/his student ID card. Registration Fee will include a Reception, Daily Tea/ Coffee between sessions and Two Lunches in the Cafeteria.   The registration fee must be paid in US dollars payable by a check drawn on a US Bank or a money order. Please make checks payable to “Fordham University EHR05”. 7]. REGISTRATION FEE Financial Support Narrative. If you believe that you need/ deserve financial support in the form of a discount / waiver in the registration costs please include a short (less than 100 words) narrative explaining your request here. For example, your narrative might include a statement that you are paying out of personal funds and not from your employer, presenting a paper, or are not planning to attend lunches or the reception, or want to waive the $5 per month surcharge for late registration, and so forth. The narrative can simply state that “I am unable or unwilling to secure funding from my employer.” 9] SIGHTSEEING: There are tremendous sightseeing options in the New York area. The conference location is next door to the world famous Lincoln Center for Performing Arts. We want the participants to enjoy their visit to New York. Print the file, include your payment (lodging fee for first night) and mail to the address above. E-mail the file with “EHR05-Registration” in the subject field. If you are presenting a paper and do not wish to contribute toward the cost of this conference by paying the registration fee, please include a short narrative to say so.
