Medical History and Physical Examination
Schedule an appointment with your doctor as soon as possible so that you can return the recommended medical form to the Student Health Center by January 18th. You can download the physical form and the required immunization form, for your practitioner to complete at www.fordham.edu/health and click on the Online Forms.
Vaccinations Required for All Freshmen
By law, New York State requires that all students born on or after January 1, 1957, demonstrate proof of immunity against measles, mumps, and rubella. Proof of immunity consists of:
Two doses of live measles vaccine, physician verified history of measles disease, or a blood test showing immunity.
One dose of live mumps vaccine, physician verified history of mumps disease, or a blood test showing immunity.
One dose of live rubella vaccine, or a blood test showing immunity.
New York State recommends a meningitis vaccination. If you refuse, you must sign where indicated on the immunization form part 2. If you would like to receive the vaccine Menactra, it is available for $95 at the Student Health Center.
