Stafford Loan Request Form
If you have already completed a Master Promissory Note you will not be required to sign additional promissory notes when requesting additional loan amounts using the same guarantor.
By completing the On-Line Loan Request, I understand;

I am requesting a total amount of subsidized and unsubsidized loan under the Master Promissory Note not to exceed the allowable maximum under the Higher Education Act. I may decline the loan or request a lower amount by contacting my lender or school. This On-Line Request acts as my confirmation of the amount for which I would like my loan processed dependent on my eligibility under federal guidelines.

  • Additional information about my right to cancel or reduce my loan is included in the borrower's rights and responsibilities statements and disclosure statements that have been or will be provided to me.
  • This is an annual process. You must refile the FAFSA annually and the loan request form for each new loan.

To request a Stafford Loan, simply complete our

On-line Loan Request Form 2008-2009. Once your loan request is submitted you will be sent to a page that says “Thank you for completing your loan request on-line.” If you do not see this page your request has not been successfully submitted. For information on choosing a lender, click here.
