Work Study Placement Forms

In order to proceed with your in-person interview, you will need to complete and print the Tax Worksheet and Tax Withholding Forms that pertain to you; consult with your parents and/or accountant since we are not able to provide tax advice. Also, please complete and bring all the forms below with you at the time of your scheduled interview. Note that Direct Deposit is optional.

1. Tax Worksheet for Student Employment (required for all)

2. Form W-4 (2008) (required by all)

3. Choose one (required by all)
a. NYS Certificate of Exemption from Withholding (Form IT-2104-E)
b. NYS Employee's Withholding Allowance Certificate (Form IT-2104)

4. Certificate of Non-residence& Allocation of Withholding Tax (Form IT-2104.1) (required if you live outside of the five boroughs of New York City or Yonkers)

5. I-9 Employment Eligibility Form (Please bring the appropriate documents with you at the time of your scheduled interview)

6. 2008-2009 Student Employment Placement Form

7. New Hire Action Form

8. Direct Deposit Application (optional for students who want their paycheck sent directly to their checking or savings account)

9. Ram Van Letter/Application/Authorization Forms (ONLY complete these forms if you are interested in becoming a Ram Van driver)
