The Immunization Form is required for all enrolled students of Fordham University per the New York State Department of Health.
The following vaccines are required by NYS Law: 2 Measles, 2 Mumps, and 1 Rubella. The meningitis vaccine is highly recommended. If the student has not received the meningitis ACWY vaccine within the past 5 years and a complete 2 dose series of meningitis B vaccine, they must sign the meningitis response form.
Fordham University will require all students who are taking in person classes, living in University housing or entering the campus for any reason to be vaccinated for COVID-19 by the opening of the fall 2021 semester.
Fordham University will require all students- undergraduates and students in graduate and professional schools-- to be vaccinated by the opening of the Fall semester (medical and religious accommodations will be considered).

Please note that the Physical Examination is highly recommended for the incoming students who have a significant past medical history, are on medications, or have a history of anaphylaxis or severe allergic reactions.
Food Allergy Forms
Food Allergy forms are highly recommended for incoming students who have a significant history of anaphylaxis or severe allergic reactions.
Allergy Injection Form
Only for students who will need to get their Allergy Immunotherapy at the University Health Services. Students should bring these required documents and their serums to University Health Services before scheduling their first appointment.
Records Release Form (only for currently enrolled students and alumni)
This form is for University Health Services' staff to release medical records to a doctor's office or another institution.
